feedback form not sent to recipient

Also, you can double check your settings in [InformationCollection] and [MailSettings] in settings/override/site.ini.append.php and settings/siteaccess/your_siteaccess/site.ini.append.php. If that doesn't work, try changing the mail transport to "file" in site.ini to troubleshoot. You can see what results are returned in var/log/mail, too. Can you gain any insight from turning on template redirection and debug output?
