How to install eZ Newsletter 1.6 on eZ Publish 4.0?

There is only page at http://ez.no/eznewsletter that touts its many features, but the setup doc at http://ez.no/doc/extensions/newsletter seems to be for something totally different and is unusable. The downloadable file is bunch of zipped ezpkg files. What to do about them? 4.0 has been the suggested eZp version for a long time, newsletter docs should be updated with essential install and configuration instructions for that version.
