Error: when trying to creat a text link

Hi ezpeople,

I keep getting this error whenever I try to browse to an eznode while trying to creat a text link

Not Found

The requested URL /insertlink was not found on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

Any suggestions on how to fix this problem?

I'm running ezpublish 4.0.1
